Description Lesions of the optic tract produce a characteristic triad of clinical findings: contralateral homonymous hemianopia, contralateral relative afferent pupillary defect, and bilateral optic disc atrophy. This case describes clinical features, radiological findings, and optical imaging characteristics for a patient who was referred to our clinic for evaluation of newly detected homonymous hemianopia who was ultimately diagnosed with optic tract syndrome secondary to lacunar infarction. This case highlights the importance of precise clinical examination in localizing and diagnosing the neurological lesion.
