Description The purpose of this Doctorate of Nursing Practice (DNP) project was to implement a self-measured/home blood pressure (SMHBP) monitoring program at one stand-alone, urban primary care clinic, offering free medical services to low income children and adults with no health insurance. The clinic is run mainly by volunteers. Diagnosing and appropriately treating hypertension reduces risk for heart attack, stroke, and other co-morbidities. SMHBP monitoring is considered best practice to improve hypertension control and reduce cardiovascular morbidity/mortality. The Clinic's goal is to improve from 50 percent hypertension control to at least 70 percent as recommended by the Centers for Disease Control and Prevention (CDC) in its Million Hearts Program. The Promoting Action on Research Implementation in Health Service (PARiHS) model, with its attention on the context for implementing evidence-based practice, guided this project.
