Medical Home Neighborhood is a web application that helps parents and guardians of children with special health care needs to easily find and discover information related to the child's condition. The users can filter information based on condition, age and category. The age filter spans from age 0 to 22, the available categories are: Support, Healthcare, Education, Mental Health, Financial and Independence.
