Description This is a single-center, retrospective, descriptive, quality improvement project. University of Utah Hospitals and Clinics (UUHC) provide antineoplastic treatment to patients from the mountain west region. Pharmacist compliance to competency policies and standards are essential for patient safety and successful health care operations. Healthcare institutions rely heavily on clinical pharmacists. Pharmacists are tasked with a unique opportunity to lead efforts to prevent medication errors. Medication errors are preventable through effective systems that involve a multi-disciplinary team. Through quality improvement measures, UUH is more compliant to the antineoplastic ordering and verification policy.
