Description The main OR has a Time out defect rate of 81% with an overall defect rate of 95. Initiation is by the surgeon 22% and 73% by the nurse. Time out is conducted by the surgeon 7% and 89% respectively. 48% of the time required information was not verified. Conversation did not cease in 29%. Other issues arose in 23%.
