Description Central nervous system cavernous malformations affect roughly 0.5% of the population and can occur anywhere in the brain and spinal cord. Cavernous malformations affecting the optic pathway are extremely rare. Patients with these lesions typically present with acute headaches and sudden loss of vision from chiasmal apoplexy. We present a case of sudden vision loss due to a cavernous malformation affecting the optic nerve, optic chiasm and optic tract. Only 9 similar cases have been reported in the literature.
