Description Patients with carotid-cavernous sinus fistulas may present with ipsilateral, contralateral, and bilateral neuro-ophthalmic fixate manifestations with respect to the side of the arterial source. We report a case of a 77-year-old woman with chronic uncontrolled severe hypertension who presented with a two month history of horizontal diplopia and bilateral visual loss.
