Description To describe the symptoms, examination and neuroimaging of two patients with spontaneous intracranial hypotension presenting as 6th cranial nerve palsy. A 36 yof and a 49 yof each reported cervical pain, postural occipital headache, and nausea, followed two weeks later by horizontal diplopia. Examination of each was normal with the exception of a right 6th cranial nerve palsy. There was no trauma.
