Description A 26 year old healthy woman presented 5 hours after painful loss of vision O.S. and frontal headache. Her visual acuity was 20/20 O.D. and light perception O.S., with a RAPD O.S. An MRI revealed diffuse enhancement of the left optic nerve sheath and sinusitis. Within 1 week, her vision deteriorated to 20/200 O.D. and NLP O.S.
